高倍率形态学和透明质酸结合在选择有完整DNA整倍体精子上没有差异

我们比较了三种精子选择方法,包括传统的,基于精子细胞器形态检查(MSOME)标准的高倍率形态学,以及精子染色体非整倍体率和DNA碎片率的透明质酸(HA)结合。精液来自于50个有严重男性因素不育的病例,经密度梯度离心后,用传统方法(对照组),高倍率(×6650)或透明质酸结合方法进行精子选择。用荧光原位杂交探针检测染色体13、18、21、X和Y的非整倍性,用末端标记法(TUNEL)检测DNA碎片。高倍率下选择精子拥有较低的DNA碎片率(2.6% vs. 1.7%; P=0.032),在非整倍体率上其与透明质酸结合法没有显著性差异(0.8% vs. 0.7%; P=0.583)。这两种方法和对照组相比均拥有更低的非整倍体率和DNA碎片率(对照组:7% 非整倍体率和26.8%的DNA碎片率)。在高倍率组中,当选择最好的精子时要比仅能选次好的精子时的非整倍体率低,但在DNA碎片率上没有差异。总之,在高倍率下选择精子可以有更低的DNA碎片率,比透明质酸结合选择精子更有效,但这一小小的差异(0.9%)可能对临床的意义不大。
