为了评估血清前列腺特异抗原对前列腺体积及下尿路症状(由IPSS 评分来表示)的预测功能,我们收集了2001 年1月至2011 年12 月首次入选韩国前列腺健康协会筛选项目的男性患者资料。血清前列腺特异抗原水平大于10 ng ml-1 或年龄大于40 岁的患者被剔除。因此,共34857 人纳入了我们的研究,检测和估算所有患者的血清PSA,PV 和IPSS 。使用线性和年龄修正的多因素Logistic 回归分析来评估PSA 与PV 或IPSS 之间的潜在联系。PSA 对估算PV 及IPSS 的预测价值通过ROC 曲线下面积来评估。平均PV 为29.9 ml ,平均PSA 水平为1.49 ng ml-1 ,平均IPSS 为15.4,PSA 与PV 之间有显著关系,在年龄修正后IPSS 评分与PSA 有显著相关性。PSA 预测PV >20 ml,25 ml 和35 ml 的AUC 分别为0.722,0.728 和0.779 。PSA 预测IPSS>7,>13,>19 的AUC 分别为0.548,0.536 和0.537 。在一项以社区为基础的大型筛选队列研究中,血清PSA 是一个很强的PV 预测指标。尽管PSA 与IPSS 也有着重要相关性,但PSA 对截止水平以上的IPSS 预测价值并不理想。需要通过进一步的前瞻性对照研究来明晰PSA 与LUTS 之间以及PSA 与PV 之间的确切相互作用。这些研究也许可以建议如何应用PSA 来临床预测PV 和IPSS 。
